You may ship the ashes to Hawaii yourself, or you may ask your funeral service provider to assist you.

If you would like we can send you the necessary forms and information via email, fax, or regular postal mail.  Just call us at 800-908-9764, and we will be happy to assist you. 

1. Please wrap the container with the cremated remains with a padded material and place all in an appropriate shipping box.   

2. Mark the outside of the shipping box on the address side with "Cremated Remains".

3. Include the following additional forms & information inside the shipping box, or attach it securely to the outside of the shipping box::

a. Shipping Form   - click here to print  form-

b. Ash Scattering Authorization Form - click here to print form - 

c.  Disposition Papers (burial permit) from funeral provider.

d. Copy of the Death Certificate

e. Any special requests

f. Payment Form ( if paying by credit card) - click here to print form - 
    ... or money order made to the sum total amount of services. 

4. Ship to the following address via U.S. Postal Service's Registered Mail with Return Receipt:
